Important information:
- Receipt of report submissions will be confirmed via email and no cost extensions will be returned after signed.
- Report submissions will include completed form and any uploaded PDF attachments.
- You can review the form in its entirety before or after you begin completing it. There is a prompt to upload the attachment at the end of the reporting forms.
- On the third input screen you will have the option to complete now or complete later. If you select complete now, you must finish and submit form in one session. If you select complete later, you will receive an email link, complete at your convenience, and save progress along the way (and access link until ready to submit).
- If you want to print the form to see all questions before completion, click the >> on top of the form. It will pull up the option to print or save the form. You can also download the form here.
